Change Membership Request Form
Members wishing to change membership types should complete and submit this form. Please indicate both the membership type you are change from and the membership type you wish to change to.
Memberships reactivating from an On-Hold status should use this form, changing from On-Hold to the desired membership type.
Change requests should be received by the 25th of the month in order to become effective on the 1st of the following month.
Once recieved and processed, the Front Desk will send an email confirmation that the change has been made, or to request additional information, if needed.
